Putting Diabetes on the M.A.P
|Developing diabetes is a scary thought! Trying to change to a healthier lifestyle, losing those extra pounds and increasing exercise is not always easy without support.|
The prevalence rate of adults diagnosed with Pre-Diabetes is rising dramatically and in England has increased from 11.6% in 2003 to 35.3% in 2011. (BMJ 2014). The Move Away from Pre-diabetes (M.A.P) programme was created by a team of Dieticians working in Ealing Hospital NHS trust as an initial pilot in 2011. The programme has now developed into an innovative, structured behaviour change initiative that is very well attended with 71% of patients completing the programme. Patients are empowered to make sustainable changes to their diet and physical activity, preventing the development of diabetes and cardiovascular disease.
Individuals who have been identified as pre-diabetic through the NHS Health Check programme can be referred onto the programme by their Primary Care Physician. They are then enrolled into a dieticianled, 6-month intensive lifestyle intervention (with an 18 month follow up programme), which provides tailored goals for weight loss and exercise. This programme is unique in that it combines dietary and physical activity intervention with both group based and individual consultations. Fundamental to the success of M.A.P is that it is delivered by Dietician and Exercise specialists trained to engage and motivate individuals with skills they need to self-manage.
M.A.P can be effectively co-produced and adapted for any setting and is a package which provides all resources, tools and information required to bring successful outcomes. The programme involves a robust process of clinical assessment which demonstrates remarkable results with 82% of patients showing a reduction in HbA1C and 66% have moved out of Pre-Diabetes. M.A.P patients average a 4.1% reduction in weight and a 4.9% reduction in waist size along with overall improved fitness upon completion of the programme.
